Each profession usually has their own code of ethics as determined by their national organization,  and the formats may vary.  For example, the human services professionals have a statement of Ethical Standards endorsed by their national organization.  Here's the link to that statement:

Formating would be optional, but should be easy to understand and in business style (formal terms, full sentences.)
